List of documents for a Polish resident card

Before you start preparing documents for a Polish resident card, make sure that you:
✅ has been legally and continuously residing in the territory of the Republic of Poland for at least 5 years immediately prior to submitting the application;

✅ has a source of stable and regular income. IMPORTANT: before the day of filing documents, make sure in ZUS that you have paid taxes for the last 3 years;

✅ speaks Polish (at least at level B1) and can provide documentary evidence of this.

The UE long-term residence card is issued for an indefinite period.

List of required documents:

1. Completed application (wniosek);
2. Photos, 4 pcs;
3. Copies of all pages of the foreign passport (with entries);
4. Document confirming place of residence (notarial act, notarized document);
5.. ZUS registration on payment of taxes for the last 3 years;^
6. Confirmation of insurance in Poland;
7. Certificate from the US on the absence of debts on the territory of Poland. 
8. A document confirming knowledge of the Polish language (minimum level B1). The certificate can only be obtained by passing egzamin telc or state exam
9. Document confirming stable, regular income (umowa o prace/zlecenia itd)
10. PIT-37 for the last 3 years with UPO confirmation
11. Administrative fee of 640 PLN

Documents that may also be requested in the future (submitted depending on the case and the inspector):
— A statement (in free form) about the number of people who share a household with you
— Marriage certificates, birth certificates of children translated into Polish Polish sworn translator.
— A statement (in free form) on exits and entries over the past 5 years.
— A statement (in free form) of continuous residence in Poland for at least 5 years at the time of filing the application (copies of visas, decisions issued by the Voivode)
